Erythrocyte band 3 profile in falciparum malaria and sickle cell disease: a comparative cross-sectional study.

Background
Band 3 is a major erythrocyte membrane protein involved in membrane structure, parasite invasion, cytoadherence, and sequestration. Alterations in Band 3 may contribute to the pathophysiology of falciparum malaria and sickle cell disease. This study evaluated the Band 3 profile in these conditions.
Methods
This single-center comparative cross-sectional study included patients with uncomplicated falciparum malaria, severe falciparum malaria, sickle cell disease, and healthy controls. Malaria was diagnosed by peripheral blood smear and/or rapid diagnostic testing, while sickle cell disease was confirmed by high-performance liquid chromatography. Erythrocyte membrane Band 3 was analyzed using sodium dodecyl sulfate-polyacrylamide gel electrophoresis and densitometry. Band 3 fragmentation was assessed as high-molecular-weight aggregates, Band 3 monomer, and proteolytic fragments.
Results
Among 71 participants, 19 had uncomplicated malaria, 22 had severe malaria, 20 had sickle cell disease, and 10 were healthy controls. Relative to controls, Band 3 densitometric values were 139% in uncomplicated malaria, 138% in severe malaria, and 171% in sickle cell disease. High-molecular-weight aggregates were increased in severe malaria and sickle cell disease, whereas proteolytic fragments were reduced. Band 3 density did not differ significantly between uncomplicated and severe malaria.
Conclusion
Falciparum malaria and sickle cell disease were associated with quantitative and structural alterations in erythrocyte Band 3.
Recommendation
Larger multicenter and longitudinal studies with molecular and functional investigations are recommended to validate these findings and determine the potential diagnostic and prognostic significance of Band 3 alterations.
